CHILD, ADOLESCENT AND FAMILY MENTAL HEALTH SERVICE

Intensive Case Management Team

A service for children and adolescents (under 18 years, or up to 20 years if under guardianship or custody of the Chief Executive of CYF), who are active and current clients of the Child, Adolescent and Family Mental Health and Child, Youth and Family Service. Clients require a greater level of input than any one service can provide, or the involvement of multiple agencies from different sectors. The Intensive Case Management Team Case Managers work with the family and services involved to facilitate and co-ordinate individualised care plans in order to create a System of Care around a young person. Referral to this service is via the Strengthening Families process.
