Competencies

The reasons for competency documents:

 

POCT must be performed as accurately, precisely and reliably as tests that are analysed in the central laboratory. A standardised training program ensures these requirements are met. Competency testing is a way of measuring the outcome of the training program.

Competencies are mandatory. CDHB Staff members who have not been trained by the POCT Coordinator or designated authority may not undertake the POCT procedure.

Failure to comply with this requirement contravenes the following policies:

  1. The CDHB POCT Policy, Clinical Manual, Vol 11, Section 1.8, Training & Competency:
  2. "All staff using POCT equipment receive training in the use of the equipment. The POCT Co-ordinator will ensure a training program is in place and there is a system for documenting when training has been given".

  3. Quality Health New Zealand, Accreditation Standards for Health and Disability Services, 2001 Version 2, published by Quality Health NZ, in March 2002.

    The Standard Service Continuum, Section SC3.0 refers to Diagnostic Services that are "...safe, efficient and accurate".  

    POCT services are mentioned specifically: Section SC3.5 refers to "…accurate and reliable diagnostic tests…", "…qualified and properly trained people carrying out the services…"…internal quality control…" and "…external quality reviews…".

  5. ISO 22870, the laboratory accreditation policy for POCT. ISO22870, Section D5.1.5 b):

"Only personnel who have completed the training and demonstrated competence shall carry out POCT. Records of training/attestation and of retraining and re-attestation shall be retained".

 

Competency testing should include but not be limited to the following topics:

  • sample quality

  • analyser operation or manual procedure

  • calibrations

  • quality control

  • interpretation of both normal and abnormal results

  • sources of error

  • regular maintenance schedules

  • health and safety issues

  • documentation

  • training, ongoing training and annual competencies

  • basic troubleshooting

  • support contacts
