Dr Smith gained clinical (FRACP) and laboratory (FRCPA) fellowships in haematology in 1994. After working in Dr Alan Nurden's platelet research laboratory in Bordeaux, he spent 7 years as Consultant Haematologist in Coagulation Medicine at the Guy's & St Thomas' Hospitals NHS Trust, in London.

In 2002 he was appointed as a consultant haematologist with the CDHB, with responsibilities in general and malignant haematology, and in haemostasis. He has an active specialty interest in Coagulation Medicine, and is lead clinician for haemostatic disorders covering both paediatric and adult services in Christchurch. He directs a research programme into the molecular diversity of FVIII-reacting B cell clones of haemophilia patients with inhibitors. He has guided development within Canterbury Health Laboratories of molecular diagnostic expertise in the haemophilias, culminating in the provision for the first time in New Zealand in 2007 of pre-implantation genetic diagnosis for haemophilia. His other interests include thrombophilia and anticoagulant strategies in pregnancy.
