On Friday 8 September, there was a failure in the last of three stages of sterilising a batch of surgical instruments.
These 12 instruments went into the Christchurch Hospital system and six were used on patients, predominantly during keyhole surgery. As soon as the problem was identified, the remaining instruments were withdrawn.
Eighteen patients have been identified as having had treatment that might have involved one of the six instruments. It is not possible to identify exactly which six patients were treated with the instruments.
Although the risk of these instruments transmitting an infection is extremely small, the Canterbury District Health Board is taking the matter very seriously.
It has apologised to all individuals involved and is undertaking a thorough investigation of the incident. Precautionary measures have been put in place to ensure that the same problem could not occur while the incident is under review.
The CDHB has advised all patients involved and will assist to provide any ongoing support that is required.
