Antiarrhythmics � local anaesthetic class

These may be useful in refractory neuropathic pain – CAUTION in patients with structural heart disease or previous MI as there is an increased risk of arrhythmias
Mexiletine - 50 mg TDS PO starting dose – increase gradually to a maximum of 800 mg daily (16 mg/kg in 3 divided doses)
Infusions of Lignocaine may occasionally be recommended.

Note: A referral to Palliative Care is strongly recommended if considering the use of either of these drugs.
